What this website collects

This site has no accounts, ads or analytics, and doesn’t set cookies of its own.

• Hosting. Cloudflare hosts the site. Like any web host, it can see your IP address and keep standard technical logs, such as which pages were requested and when.
• Fonts. The site’s fonts come from Google Fonts, so your browser requests them from Google, which can see your IP address.
• Feedback form. If you send feedback, Formspree receives what you type, keeps a copy, and emails it to Formulary. If you leave your email, it’s used only to reply. Only what you choose to enter is collected. Please don’t include medical details.

How feedback is used

Feedback is used only to improve Formulary and, if you left a way to reach you, to reply. It isn’t sold or shared for marketing.
